New Patient Registration
If you wish to register with the practice we will ask you to fill in a questionnaire for each family member and an appointment will be made for you to see the healthcare assistant for an introductory check-up.
This is an opportunity for you to become familiar with the surgery and for us to assess your medical needs. Please let us know as soon as possible of any change of address or telephone number. In an emergency this can be vital.
For administrative purposes you will be registered with Dr D Kelly but you are free to consult with any of the clinicians.
Medical treatment is available from the date of registration. Please contact reception for further information.
How to Register
To register with the surgery you will need to complete a (GMS1), a surgery registration form which you can collect from reception or download using the links below.

New Patient Questionnaire
You will not be formally registered until you have brought these into the surgery and we have seen the following identification.:
- Photo identification such as your passport of driving license
- proof of your address, such as a recent utility bill (Gas, Electric, Water or landline phone bill or council tax When you register you will also be offered a 'new patient' medical check. This is because it can take a considerable time for us to receive your medical records and will help us understand your current medical needs.
